چکیده

Calcium Dynamics and Ventricular Fibrillation To the Editor: Warren et al1 recently performed an optical mapping study in blood-perfused pig hearts. They concluded that their results did not support the presence of nonvoltage-gated intracellular calcium (Cai) transients during ventricular fibrillation (VF) and suggested that action potential (AP)/Cai transient dissociation is a consequence rather than a cause of wave fragmentation. A major piece of evidence in support of this conclusion is the failure of BAPTA-AM, a calcium chelator, in preventing VF in these pig hearts. These latter experiments were performed by perfusing the heart with 25 mg of BAPTA-AM in 1.5 L of Tyrode solution over a 10-minute period. The hearts were then blood-perfused, and the studies on VF induction were performed. BAPTA-AM is known to incorporate into tissues slowly, and 20 to 60 minutes of infusion are generally needed before testing the antiarrhythmic effects.2,3,4 To test whether or not the duration of BAPTA-AM infusion affected its antifibrillatory effects, we performed a study with hearts harvested from 4 New Zealand White rabbits (3.5 to 4.6 kg). The hearts were Langendorffperfused with oxygenated Tyrode solution equilibrated with 95% O2 and 5%CO2 to maintain a pH of 7.4 0.05 at a rate of 35 to 45 mL/min. The coronary perfusion pressure was regulated and maintained at 70 to 80 cm of H2O. We infused BAPTA-AM 20 mol/L for 30 minutes (N 2) and for 70 minutes (N 2) before the commencement of the rapid-pacing protocol. The results showed that BAPTA-AM infusion for 30 minutes failed to suppress either Cai alternans or VF induction during rapid pacing. However, after 60 minutes of BAPTA-AM infusion, neither Cai alternans nor VF was inducible. The only arrhythmia inducible was sustained monomorphic ventricular tachycardia. In the present study, we confirmed that a short duration of BAPTA-AM infusion did not prevent VF in rabbit hearts in vitro, consistent with that reported by Warren et al.1 However, alternans and VF were no longer inducible after 70 minutes of BAPTA-AM infusion. These latter findings confirmed the antifibrillatory effects of BAPTA-AM as reported by Billman and colleagues.2,3 The results also indicate that the duration of BAPTA infusion (10 minutes) in the study by Warren et al1 may be insufficient to test the importance of Cai dynamics in the mechanisms of VF. A new approach to detect Life threatening cardiac arrhythmias using Sequential spectrum of Electrocardiogram signals

This study evaluates the discriminative power of sequential spectrum analysis of the short-term electrocardiogram (ECG) time series in separating normal and subjects with life threatening arrhythmias like, ventricular tachycardia/fibrillation (VT/VF). The raw ECG time series is transformed into a series of binary symbols and the binary occupancy or relative distribution of mono-sequences (i.e. ...

Spectral and correlation analyses of the verapamil-induced conversion of ventricular fibrillation to tachycardia in isolated rat hearts.

Ventricular tachycardia (VT) is considered to be the most common precursor of ventricular fibrillation (VF). However, the mechanisms underlying the transition from VT to VF remain unclear. Here, we investigated whether and how perfusion of the heart with verapamil, a blocker of L-type calcium channels, changed the macro-dynamics of the heart between VT and VF. The experiments were performed wit...

Modulation of extracellular atrioventricular node field potential pattern and ventricular rhythm by morphine in experimental atrial fibrillation in isolated rabbit heart

Introduction: Endorphins are produced by cardiomyocytes, and exert different effects on the heart. The aim of the present study is to assess morphine effects on extracellular atrioventricular (AV) node field potential pattern and ventricular rhythm of isolated rabbit heart during experimental atrial fibrillation (AF). Methods: Effects of different concentrations of morphine (10, 20, 50 and 1...

New concepts in atrial fibrillation: neural mechanisms and calcium dynamics.

Atrial fibrillation (AF) is a complex arrhythmia with multiple possible mechanisms. It requires a trigger for initiation and a favorable substrate for maintenance. Pulmonary vein myocardial sleeves have the potential to generate spontaneous activity, and this arrhythmogenic activity is surfaced by modulation of intracellular calcium dynamics. Direct autonomic nerve recordings in canine models s...

A localized meshless approach for modeling spatial-temporal calcium dynamics in ventricular myocytes.

Spatial–temporal calcium dynamics due to calcium release, buffering and re-uptaking plays a central role in studying excitation–contraction (E–C) coupling in both normal and diseased cardiac myocytes. In this paper, we employ a meshless method, namely, the local radial basis function collocation method (LRBFCM), to model such calcium behaviors by solving a nonlinear system of reaction–diffusion...
